Please arrive 15 minutes early to your appointment with your insurance cards, list of medications, and if needed, we may ask for a urine sample.

The providers will customize a treatment plan based on your individual case whether it be a medication change, a lifestyle modification, and generate a comprehensive care plan with your primary care physician.
after your visit
Please allow time for any requested labs to be done before your next visit.
For any refill of medication request, please give the providers time to approve and send the refill back to your choice of pharmacy.
